Medical Forms
In 2009, Boy Scouts of America consolidated their old Class 1, 2 and 3 medical forms into
a single form that is to be used for all events and all ages, youth and adults alike.
This new form has three sections that are described in detail on page 1 of the download.
To summarize, parents fill out parts A and C for all events, and a doctor must fill out
and sign part B, but only for events over 72 hours.
